Define the handoff, not just the intention

For each recurring stage, identify the required information, the person responsible and the next action. A template can help deliver a consistent message, but someone still needs to check whether it fits the actual situation. Make exceptions visible instead of silently working around them.

Review the process when something fails

Ask what information was missing, whether the step was clear and whether the team had the resources to complete it. A missed step can reveal a training need, an unrealistic workflow or a new situation the process does not cover. Update the process when the lesson is useful.

Can a written process prevent every problem?

No. It helps coordinate routine work and identify gaps, while unusual situations still require judgment and communication.
